About Our Services

Who does The Charles Henry Center serve?

We operate two programs. Our Behavioral Health Program serves children and adolescents with behavioral, emotional, and mental health needs. Our IDD & Autism Program serves adults ages 18+ with intellectual/developmental disabilities and autism.

What services do you provide?

For youth: Level III residential services, behavioral support, clinical and therapeutic support, community integration, crisis prevention and stabilization, and transition and discharge planning. For adults: Alternative Family Living (AFL), residential supports, Long-Term Community Supports (Levels 1–5), Community Networking, Community Living & Supports, respite, and community integration. What is Family Centered Treatment (FCT)?

Eligible members receiving Level III Residential Services may also participate in Family Centered Treatment to strengthen family relationships and support successful transitions home.

What are LTCS Levels 1–5?

Long-Term Community Supports are individualized supports based on each member’s assessed needs and level of support.

Referrals & Placement

Who can submit a placement inquiry?

Care coordinators, guardians and legally responsible persons, DSS/social workers, hospitals and facilities, and other referral partners can use our Placement & Services Inquiry form.

Does submitting an inquiry guarantee placement?

No. All referrals are reviewed to determine whether our programs can appropriately meet the individual’s needs and whether the requested service is available.

Should I include clinical records with the inquiry?

No. Clinical records and supporting documentation should be submitted separately, as instructed by The Charles Henry Center. Additional documentation may be requested during the referral review.

My Medicaid plan or LME-MCO isn’t listed. Can I still submit?

Yes. The form lists Alliance, Partners, Vaya, and Healthy Blue, and includes an “Other” option. Your inquiry will be reviewed either way.

How are urgent placement needs handled?

The inquiry form asks whether the need is urgent and whether the service is currently authorized. Mark urgent needs clearly so our team can review them promptly.

Privacy

How is my information protected?

The Charles Henry Center is committed to protecting the privacy and confidentiality of the individuals we serve. Information submitted through our forms is used solely to review the referral, determine eligibility and availability, and coordinate next steps, and is handled in accordance with applicable privacy laws, including HIPAA when applicable.
